Proof of Disability

"Proof of Disability" is formal documentation of a disability that conforms to the Americans with Disabilities Act or to Section 504 of the Rehabilitation Act.

Documentation should come from a qualified professional such as a physician, psychologist, psychotherapist, educational psychologist, learning disability specialist, audiologist or neuro-psychologist.

Sample Proof of Disability Documents

  • For learning disabilities: An Educational/Psychological Evaluation Report (with diagnosis).
  • For attention deficit disorders: A physician's statement of diagnosis or a psychologist's report.
  • For deaf/hard of hearing: An audiogram.
  • For physical impairment: A physician's statement of diagnosis.
  • For visual impairment: A physician's statement of diagnosis.
  • For emotional impairment: A statement of a specific diagnosis from a qualified professional.
  • For neurological impairment: A physician's statement of diagnosis.
